The Hight Support Programs primary goal is to facilitate a healthy and supportive environment that maximizes successful outcomes during the transition to independent living from a hospital setting. With two Brampton locations, our model of long term housing with an increased level of on-site support for all clients is designed to assist individuals who are challenged with complex mental health and co-occurring issues.
Our team provides emphasis on recovery and rehabilitation, and all tenants are assigned Healthy Housing™ accommodations, access to recreational activities and transportation, nursing support, and 24/7 individualized support models which aim to:
- Facilitate socialization activities and practices
- Encourage healthy and sustained community integration
- Monitor client progress and ensure the safety of program participants
Onsite intensive support is provided by a team of Community Mental Health professionals, Registered Nurses, Recreational Therapists, SHIP North Peel ACTT team members, and the CAMH Forensic Outpatient Team. The High Support Team provides clients with support in activities of daily living – including dietary and hygiene support, overall wellness, recovery goal planning, and community referrals and linkages.
