An evidence-based initiative that was created to enhance health care services for older adults in Ontario with complex and responsive behaviours.

Our Behavioural Supports Ontario – Community Crisis Worker aims to improve the lives of older adults and their families by supporting older adults with complex and responsive behaviours associated with dementia, mental health, substance use and/or other neurological conditions across the Central West LHIN. A Community Crisis Worker provides timely crisis assessments, mobile visits and brief crisis management services to enable and support older adults with responsive behaviours and their caregivers in the community. Support is offered through a collaborative and shared service delivery model across the continuum of care. The Community Crisis Worker, works in partnership with the SHIP Integrated Seniors Team and the 24.7 Crisis Support Program Peel through the Canadian Mental Health Association Peel Dufferin.
