As the lead organization and backbone support of the Housing First Collaborative program, comprised of 6 partner organizations, we offer intensive case management, housing support, and portable housing allowance.
Home For Good Peel offers program management and coordination as well as a secondary support team for direct support and consultation. Our service pathway takes a new approach to prioritization following housing first best practices. Referrals are triaged via our community’s “ By Name” List, considering the factors of chronicity, acuity, tri-morbidity and vulnerability. Following intake assessment, clients are assigned to one of the five collaborative partners offering intensive case management services to program participants:
