In order for SHIP to proceed with your application, it is important that we have your consent. The last part of the application is where you will confirm your declaration of consent which follows and adheres to the Personal Health Information and Protection Act (PHIPA). If this is someone applying on behalf of another person, there is a Referrer’s Declaration at the end of the application that must be completed which is in compliance with PHIPA.
