The Residential Multi-Service Team assists individuals living in the Peel Region by providing services specific to the following needs: Case Management (Dufferin County) General Mental Health and Seniors, Dual Diagnosis and Mental Health and Justice.

General Mental Health

16 + who have a serious mental illness and who require assistance with life skills training and who would benefit from supportive counseling and health training. Clients are housed in the Region of Peel.

Seniors

Supportive case management is provided to clients who are 55+ residing in the Region of Peel. Counsellors are experienced in managing age related issues, providing linkages to appropriate community resources.

Case Management

This program is specific to individuals 16 + living in the County of Dufferin. Case management supports include assistance with life skills training, supportive counselling and health training as well as providing linkages to community resources.

Dual Diagnosis

This program provides individualized support to clients who are 16+ and have a developmental disability and a serious/persistent mental illness, are homeless or at risk of homelessness. Counsellors are knowledgeable on dual diagnosis and work collaboratively with community resources to ensure clients live independently in their community.

Mental Health and Justice

This program provides support to individuals 16+ who are homeless, inadequately housed or are hard to house, have been in contact with the law and have psychiatric/medical needs to live independently in the Region of Peel. Counsellors have knowledge of the Criminal Justice system and mental illness and provide case management and supportive counselling.

How To Apply

Referrals for the General Mental Health, Dufferin Case Management, Dual Diagnosis and Justice and Mental Health programs are submitted through Central Intake.

Assertive Community Treatment Teams (ACTT)

Our supportive service is available to individuals between the ages of 16- 65 who are experiencing great difficulty in accessing or going into the healthcare setting.
