Dufferin Case Management

Offers flexible case management support for individuals who are 16+ experiencing mental health concerns residing in Dufferin County.

Case management offers recovery oriented services which are client driven and supported by a Community Mental Health Counsellor.

Services include intensive case management service for individuals aged 16+ who reside within Dufferin County.

How To Apply

Referrals are processed through the SHIP Central Intake Department.

Our supportive service is available to individuals between the ages of 16- 65 who are experiencing great difficulty in accessing or going into the healthcare setting.
