The In-STED (In Short-Term Emergency Diversion) program aims to identify and support complex mental health and addictions needs through early intervention. The program operates a short-term case management service that is high quality, person-guided while ensuring to wrap around cultural humility and inclusivity.

The intended outcome is to reduce unplanned repeat visits to hospital emergency departments and/or effectively support individuals while awaiting services in the community. The average length of service will be 3-6 months (with a focus on shorter-term when possible).

Eligibility:

  • Resides within Peel (Mississauga and Brampton) or Dufferin County
  • 16 years of age or older
  • Mental Health and/or Substance use concerns self identified or formal diagnosis
  • Individuals at risk of attending hospital ED or visit/admission within the last 90 days
  • Goals or support needs in line with community case management
  • Not already receiving case management support
