The Integrated Seniors team is a partnership program between SHIP, Peel Senior Link (PSL) and Punjabi Community Health Services (PCHS) serving the Brampton community.

The integrated seniors team is a program designed to support older adults 55 years of age or older. Clients generally exhibit complex physical, emotional and medical concerns and may also be without appropriate housing.

Our service strategies include: in-home community counselling support through SHIP and PCHS and personal care through PSL, addressing issues of isolation and loneliness, conducting assessments, providing education and support, assisting with personal support services, crisis prevention, falls prevention education and intervention. The goal of this program is to reduce hospitalizations and /or ER visits as well as reduce the need for Alternative Level Care beds and premature long-term care.
