Mental Health Consultation

The Mental Health Consultant is an integrated service to the LHIN Home and Community Care program that supports Care Coordinators in connecting individuals receiving services and present with mental health concerns to appropriate community resources and services.

How To Apply

Referrals are made directly from Home and Community Care Coordinators to the Mental Health Consultant.

Our supportive service is available to individuals between the ages of 16- 65 who are experiencing great difficulty in accessing or going into the healthcare setting.
