The Supportive Housing Teams aim to provide supportive housing and intensive case management for people with serious mental health issues and/or those with problematic substance use who are homeless or at risk of homelessness.

The Supportive Housing Teams include Team Mississauga, Team Brampton, and the Dufferin Services Team, with the latter also providing service in Caledon.

This multidisciplinary group (including specialists in Alternative Dispute Resolution, General Mental Health, Hoarding, Dual Diagnosis, Addictions, Justice, Vocational and Seniors) utilizes a Housing First approach, and includes a combination of resources and supports:

  • Individualized assessment and coordinated care planning
  • Matching individuals to appropriate housing / choice based housing
  • Head lease units or rent supplements
  • Engagement with clients to access and optimize primary care
  • Assistance with activities of daily living, support services and crisis avoidance
  • Structured and unstructured groups will be facilitated; offering support, conflict resolution, eviction prevention strategies and resident input into their housing environment.
